On average, 7.5 million medication errors happen in the pediatric population yearly (NIH, 2022). Studies show that 14-31% of pediatric medication errors result in harm or death (D'Errico, 2022). Medication errors are defined as any avoidable event in prescribing, transcribing, dispensing, administering, or monitoring, regardless of the occurrence of injury or potential injury (D' Errico, 2022).
