Abstract
Annually, 7,000 to 9,000 Americans die from medical errors. More than half of all medication administration is done so incorrectly. Medication administration errors are an issue seen worldwide. The main reasons for medication errors are inadequacies in checking systems and errors at different stages going undetected. The five rights of medication administration were made to be the standard of practice for decreasing patient harm.
