SMART Goals for Improving Health Outcomes

Abstract
Introduction: Health and Hope Clinic (HHC) serves to provide free healthcare to the uninsured and medically underserved populations in Escambia County. The identified need was patients lack of knowledge about how to set health goals. Research findings show individuals who set SMART goals have positive behavioral changes and improved health outcomes.
Aims: The purpose of this project is to educate clients in the HHC Food as Medicine program about setting goals to improve health. The learning objectives are: (1) Discuss the importance of setting goals to improve health; (2) Identify appropriate health promotion activities; and (3) Develop one SMART (specific, measurable, attainable, realistic, and time oriented) health goal.
Methods: The group created an infographic to explain how to create a SMART health goal. A survey was created in both print and digital (Google Forms) formats, providing step-by-step tools for development of SMART health goals.
Results: The group implemented the deliverables with 10 participants at HHC with 7(70%) creating a SMART goal and 3(30%) refusing. The seven participants were able to create a SMART goal using appropriate HHC activities to achieve their goal and discuss the importance of goal setting. The digital form was preferred over the print format.
Conclusion: When people are faced with multiple comorbidities it can be difficult to know how to create goals to improve health. SMART goals are cost-effective ways to improve client health outcomes. The hope of this project is to empower the patients at HHC to achieve positive behavioral changes.
