Abstract

Infants born before 28 weeks’ gestation, often referred to as extremely preterm infants, face significant challenges related to survival and long-term outcomes. Survival rates were 24.1% for preterm infants born before 24 weeks. This is a staggering difference when compared to the 71.6% survival rate in preterm infants born after 24 weeks. (Qattea et al. 2022) Neonatal resuscitation plays a key role in their immediate management. Advances in neonatal intensive care, including mechanical ventilation, surfactant therapy, and specialized neonatal resuscitation methods, have improved outcomes (Silva et al. 2023). However, these interventions are not uniformly applied as the current standard for all extremely preterm infants, as families are frequently confronted with difficult decisions regarding whether to initiate full neonatal resuscitation or withhold or withdraw life-sustaining interventions (Arimitsu et al. 2025) Immediate resuscitation decisions can have lifelong effects on an infant’s survival (Arimitsu et al. 2025), however, concerns remain regarding potential long-term complications, such as developmental delays, cerebral palsy, chronic lung disease, and visual, hearing, and cognitive impairments (Arimitsu et al. 2025). Understanding potential benefits related to full neonatal resuscitation and complications helps evidence-based practice and clinical decision-making.
