Postpartum hemorrhage (PPH) remains one of the leading causes of maternal morbidity and mortality worldwide, accounting for approximately 25% of maternal deaths and disproportionately affecting women in low- and middle-income countries and regions with a low socio demographic index (Salati et al, 2019). These disparities highlight the importance of evidence-based prevention strategies, particularly during the third stage of labor, which is defined as the period between the birth of the infant and the delivery of the placenta. During this stage, uterine contractions promote placental separation and compress maternal blood vessels to minimize blood loss. The World Health Organization (WHO) defines PPH as blood loss of 500 mL or more within the first 24 hours after birth. Management of the third stage of labor may be either expectant, allowing spontaneous placental delivery, or active, which involves the administration of a uterotonic medication, controlled cord traction, and delayed umbilical cord clamping when appropriate. Active management of the third stage of labor (AMTSL) has consistently been shown to reduce the incidence of postpartum hemorrhage by nearly 49% , decrease severe blood loss (blood loss ≥1,000 mL) by 41%, and improve maternal outcomes, making it the recommended standard of care in most clinical settings (Salati et al, 2019).
