Abstract
Introduction: St. Joseph s Medical Clinic (SJMC) provides healthcare to underserved members of the community who are uninsured and have incomes less than $2,500 a month. There is a high prevalence of people with diabetes who utilize the clinic. People who are diabetic and poorly manage their disease are more susceptible to an increased risk of adverse health outcomes.
Aims: The purpose of this project is to create educational materials SJMC about diabetes self-management. After the education participants will be able to: 1) Describe how to properly perform diabetic foot care; and 2) Identify healthy lifestyle choices such as nutritious food options and adequate exercise routines.
Methods: The group created an pamphlet titled Diabetes Self-Management in English and Spanish using Canva. The document outlined foot care, diet, and exercise tips. Non-technical language was used in the pamphlet to address health literacy concerns.
Results: The group presented educational pamphlets to the healthcare providers at SJMC for feedback. The feedback from the healthcare providers was incorporated into the final version that was presented to 10 patients at SJMC. Participants included both patients diagnosed with diabetes and family members. Feedback was positive from the participants.
Conclusion: Diabetes self-management is critical to improve the health outcomes for the 29.1 million Americans who cope with the disease. The group hopes the developed materials will improve the health of the people with diabetes at SJMC.
